No trial has directly compared Foundayo 17.2 mg with oral semaglutide 25 mg. Lilly’s September 29, 2026 report of greater weight loss and A1C reduction is an indirect comparison of two separate trials in adults with type 2 diabetes. Novo Nordisk’s own indirect comparison in adults with obesity, released in April 2026, favored oral semaglutide. The only randomized head-to-head trial, ACHIEVE-3, tested Foundayo against 7 mg and 14 mg oral semaglutide, not 25 mg.
Foundayo (orforglipron) is FDA-approved for weight management, not for type 2 diabetes. The FDA approval announcement is dated April 1, 2026.
How is Foundayo different from oral semaglutide?
Foundayo is not semaglutide and not tirzepatide. Its active ingredient is orforglipron. Lilly describes it as a once-daily small molecule (non-peptide) oral GLP-1 receptor agonist. Tirzepatide is the ingredient in Lilly’s injectable Zepbound. The practical difference is how you take it.
| Feature | Foundayo (FDA label) | Wegovy tablets (Novo Nordisk label) |
|---|---|---|
| Active ingredient | Orforglipron | Semaglutide |
| How to take it | Once daily, with or without food; swallow whole | Once daily on an empty stomach in the morning with up to 4 ounces of water; wait at least 30 minutes before food, drinks or other oral medicines |
| Dose steps | 0.8 mg, then 2.5 mg, then 5.5 mg, with at least 30 days at each step; then 9, 14.5 or 17.2 mg based on response and tolerability | 1.5 mg for 30 days, then 4 mg, 9 mg, and 25 mg from day 91 |
| Maximum or maintenance dose | 17.2 mg | 25 mg |
| Missed dose | Take it as soon as possible; do not double up. After 7 or more consecutive missed doses, restart escalation at a lower dosage | Skip it and take the next dose the following day |
| Boxed warning | Thyroid C-cell tumors | Thyroid C-cell tumors |
The Wegovy label says its tablets contain an absorption enhancer, SNAC. Our page on why one pill needs fasting explains the difference in more detail.
Is there a head-to-head trial of Foundayo and oral semaglutide?
There is one, but at lower semaglutide doses and in type 2 diabetes. ACHIEVE-3 (NCT06045221) was a randomized, open-label, 52-week trial of 1,698 adults with type 2 diabetes inadequately controlled on metformin. Lilly says participants were assigned 1:1:1:1 to Foundayo 9 mg or 17.2 mg, or oral semaglutide 7 mg or 14 mg. Lilly says it was published in The Lancet.
| Week 52, efficacy estimand (Lilly) | Semaglutide 7 mg | Semaglutide 14 mg | Foundayo 9 mg | Foundayo 17.2 mg |
|---|---|---|---|---|
| A1C change | -1.1% | -1.4% | -1.9% | -2.2% |
| Weight change | -3.7% | -5.3% | -6.7% | -9.2% |
| Stopped due to adverse events | 4.5% (7 mg) and 4.9% (14 mg) | 8.7% (9 mg) and 9.7% (17.2 mg) | ||
Two limits matter. The efficacy estimand describes results as if everyone had stayed on treatment. Lilly also says weight for Foundayo 9 mg versus semaglutide 14 mg was not controlled for family-wise type 1 error. In the same trial, more Foundayo participants stopped for adverse events, according to Lilly’s June 8, 2026 release.
Why is Lilly’s 17.2 mg versus 25 mg report indirect?
Foundayo 17.2 mg and oral semaglutide 25 mg were never in the same trial. Lilly’s September 29, 2026 release says the analysis used Foundayo data from ACHIEVE-3 and oral semaglutide 25 mg data from PIONEER PLUS, a separate trial published in The Lancet in 2023. People were not randomized between those two arms. Statistics can adjust for differences between the trial populations, but only for the factors the analysts chose.
That is the difference from ACHIEVE-3, where chance decided who got which drug.
Here is what Lilly reports:
- Foundayo 17.2 mg was associated with 1.5% more weight loss (95% CI -2.7% to -0.2%) and 0.3% more A1C reduction (95% CI -0.6% to -0.1%) at 52 weeks.
- The main analysis was adjusted for gender, age, baseline A1C and baseline weight.
- Other statistical methods gave 1.5% to 2.4% more weight loss and 0.3% to 0.6% more A1C reduction.
- All figures use the efficacy estimand.
The weight interval runs from 0.2% to 2.7%, so the size of any gap is uncertain. The release’s headline says Foundayo showed greater results. A Lilly executive quoted in the same release says that head-to-head trials give the best comparison and that Foundayo “may deliver A1C and weight lowering that is similar to the 25 mg dose of oral semaglutide.” Lilly’s footnote also says a 25 mg once-daily dose of oral semaglutide is not approved in the United States for type 2 diabetes, and that Novo Nordisk has applied for it.
The comparison appears to have been presented at the European Association for the Study of Diabetes meeting. The release lists no journal publication for it, so peer-reviewed publication is not yet known.
What did Novo Nordisk’s indirect comparison find?
Novo Nordisk’s April 2, 2026 release describes the ORION analysis. It compared oral semaglutide 25 mg (OASIS 4) with orforglipron 36 mg capsules (ATTAIN-1) in adults with obesity or overweight without diabetes. Novo says the 36 mg capsule used in phase 3 equals the approved 17.2 mg tablet.
| Result (Novo Nordisk) | Figure |
|---|---|
| Weight difference, all patients regardless of staying on treatment | -3.2 percentage points favoring semaglutide (95% CI -5.9 to -0.4) |
| Weight difference, if all stayed on treatment | -3.0 percentage points (95% CI -5.8 to -0.3) |
| Odds of stopping for any adverse event, orforglipron vs semaglutide | OR 4.1 (95% CI 1.3 to 13.0) |
| Odds of stopping for gastrointestinal adverse events | OR 13.9 (95% CI 2.0 to 96.0) |
Novo itself says unaccounted factors may remain, trial protocols differ, and the tolerability differences should be read with caution because adverse event counts were low. The very wide intervals show that. A physician quoted in the release says no head-to-head trials exist for oral semaglutide in obesity versus orforglipron.
The two sponsors’ analyses use different trials, populations and outcomes. Neither settles which drug is better.
How much weight can I lose on Foundayo?
The FDA label reports results from two 72-week, randomized, placebo-controlled trials. All participants received lifestyle counseling. Results below are average percent change from baseline.
| Week 72 | Placebo | 5.5 mg | 9 mg | 17.2 mg |
|---|---|---|---|---|
| Trial 1 (no diabetes), weight change | -2.1% | -7.4% | -8.3% | -11.1% |
| Trial 1, lost 10% or more | 13% | 32.5% | 39.8% | 54.5% |
| Trial 2 (type 2 diabetes), weight change | -2.5% | -5.1% | -7.0% | -9.6% |
| Trial 2, lost 10% or more | 9.2% | 22.5% | 31.1% | 45.6% |
The label says missing data were imputed. Averages hide a spread: not everyone reaches the same result. In Trial 1 at 17.2 mg, 71.5% lost at least 5% of body weight, which means a meaningful share did not.
Foundayo vs the Wegovy pill: which is more effective?
Nobody knows from a direct trial. The supplied labels do not give a weight-loss table for Wegovy tablets. Two telehealth sites, Klinic and Eden, describe the 64-week OASIS 4 trial. Klinic reports 16.6% average loss among participants who fully adhered. Eden reports about 17% among those who stayed on treatment and about 14% in the overall study population.
The Foundayo figure of 11.1% comes from a 72-week trial of all randomized patients. Comparing 11.1% with 16.6% mixes different measuring rules. Klinic and Eden both note that the studies differed and that direct comparison is limited. Because the trials differ in length, population and method, the gap should not be read as established.
Wegovy pill vs Foundayo side effects
Both labels warn that adverse reaction rates from different trials cannot be directly compared. With that in mind, here is what each source reports for the top Foundayo dose.
| Measure | Foundayo 17.2 mg (label, pooled trials, placebo in brackets) | Wegovy tablets 25 mg |
|---|---|---|
| Nausea | 35% (10%) | About 47% (Eden, OASIS 4) |
| Vomiting | 24% (4%) | About 31% (Eden, OASIS 4) |
| Diarrhea / constipation | 25% (11%) / 24% (9%) | Not given in the supplied label excerpt |
| Severe GI reactions | About 3% (1%) | 2% (0%), Wegovy label |
| Stopped because of adverse reactions | 10% (3%) | 6.9% (5.9%), Wegovy label, 204-patient trial |
The Foundayo label lists hair loss at 5% versus 2% on placebo, and says it was associated with weight reduction. It also lists these warnings: pancreatitis, acute kidney injury from dehydration, hypoglycemia, gallbladder disease and lung aspiration during anesthesia. The Foundayo label tells women using oral contraceptives to switch to a non-oral method or add a barrier method for 30 days after starting and after each dose increase. It also says to keep simvastatin at 20 mg or lower. The Wegovy label reports levothyroxine exposure rose 33% with the tablet. Talk to your clinician about interactions. See also our page on Foundayo side effects.
Does oral semaglutide work as well as the injections?
The Wegovy label says that in people without type 2 diabetes, average semaglutide blood concentrations were similar after 25 mg tablets daily and 2.4 mg injections weekly. In people with type 2 diabetes, concentrations after the tablets were lower, and the label notes higher variability with tablets in all groups. It says some people with type 2 diabetes may have subtherapeutic concentrations and suggests considering the injection if response is inadequate. It also says the tablets have not been studied for weight reduction in adults with type 2 diabetes. Switching between forms is a decision for your prescriber.
Is Foundayo as effective as Zepbound, or how does it compare with tirzepatide?
The supplied evidence has no head-to-head trial of Foundayo and Zepbound. Lilly, which makes both, describes Zepbound as an injectable containing tirzepatide. Different trials, populations and durations make any number-to-number comparison unreliable. Our page on what separate studies can and cannot compare goes through it.
Is Foundayo approved for type 2 diabetes?
No. The FDA approval letter for NDA 220934 covers reducing excess body weight and maintaining weight reduction in adults with obesity, or with overweight plus at least one weight-related comorbid condition. In its June 8, 2026 release Lilly said it planned to submit for type 2 diabetes by the end of the second quarter. The supplied material does not show an FDA decision on that indication, so that remains unknown. Our Foundayo type 2 diabetes page covers the ATTAIN-2 data.
What does ACHIEVE-4 show, and what does it not?
ACHIEVE-4 compared Foundayo with insulin glargine, not with semaglutide. Lilly’s September 30, 2026 release reports 2,749 adults with type 2 diabetes and increased cardiovascular risk. Foundayo met the non-inferiority criterion for MACE-4 (hazard ratio 0.84; 95% CI 0.59 to 1.20; the prespecified limit was an upper bound below 1.8).
Non-inferiority means Foundayo was not shown to be worse than glargine by more than that margin. It does not mean no cardiovascular risk. Lilly also reports lower cardiovascular death (HR 0.47) and all-cause death (HR 0.43). Lilly’s endnotes say these were not controlled for family-wise type 1 error, and the trial was open-label. Treat them as leads, not proven benefit.
The FDA approval letter requires Lilly to complete ACHIEVE-4 and submit adjudicated MACE and potential liver-injury data. It lists a report due in July 2026. That is a standard postmarketing requirement to look for unexpected serious risks, not a detected problem. The supplied material does not show whether the FDA has reviewed the final report, so a sponsor release alone does not show the requirement is met.
What does Foundayo cost, and will Medicare cover it?
Klinic’s June 16, 2026 article lists LillyDirect self-pay prices of $149 a month for 0.8 mg, $199 for 2.5 mg, and $299 for 5.5 mg through 17.2 mg. It lists the Wegovy pill at $149 for lower doses and $299 for 25 mg. Prices can change, so check with the seller.
CMS says its Medicare GLP-1 Bridge runs July 1, 2026 through December 31, 2027 for eligible Part D beneficiaries and certain GLP-1 drugs, with a $50 copay outside the Part D benefit. The CMS Bridge page does not name Foundayo in the text supplied here. See Foundayo and Medicare for what CMS has and has not confirmed.
Why am I not losing weight on Foundayo?
This page cannot diagnose a cause. The label offers some context. Dose increases happen at least 30 days apart, based on response and tolerability. Strong or moderate CYP3A4 inducers can lower Foundayo exposure, and the label says to monitor effectiveness with moderate inducers. Trial results are averages, and not every participant reached 5% or 10% loss. Take this to your prescriber rather than changing the dose yourself.
Reviews, Reddit and celebrity claims
On Drugs.com, Foundayo averages 7.2 out of 10 from 49 ratings, and semaglutide 6.9 from 3,172, with 53% and 56% of reviewers reporting a positive effect. Drugs.com says side effects in user reviews are not clinically verified, and the semaglutide figure covers all its brands. This page does not draw on Reddit threads. The supplied material says nothing about what medication Kelly Clarkson took, so that is not known here.
